Understanding D&D 5e Conditions

Conditions in D&D 5th Edition are status effects that alter a creature's capabilities during gameplay. There are 15 conditions in the core rules, ranging from minor hindrances like Prone to devastating effects like Petrified. Knowing what each condition does is essential for both players and Game Masters.

The Most Common Conditions

Prone comes up constantly — any time a character is knocked down, they must spend half their movement to stand. While prone, melee attacks against them have advantage but ranged attacks have disadvantage. Frightened is another frequent condition, giving disadvantage on ability checks and attacks while the source of fear is visible. Poisoned is straightforward but punishing: disadvantage on all attacks and ability checks.

Conditions That Cascade

Some conditions include others. A Paralyzed creature is also Incapacitated, meaning it can't take actions or reactions on top of being unable to move or speak. Unconscious includes Incapacitated too, plus the creature drops prone and drops what it's holding. Petrified is the most extreme — the creature is transformed to stone with resistance to all damage.

Exhaustion: The Special Condition

Unlike other conditions, Exhaustion has six cumulative levels. Each level stacks with the previous ones: Level 1 gives disadvantage on ability checks, Level 2 halves speed, and it escalates until Level 6 causes death. Exhaustion is harder to remove — typically requiring a long rest to reduce by just one level.

Tips for Tracking Conditions

  • Mark conditions visibly — use tokens, cards, or a digital tracker like this one so everyone remembers.
  • Check saves each turn — many conditions allow a saving throw at the end of each turn. Don't forget!
  • Stack effects carefully — a Stunned creature is also Incapacitated. Apply both sets of rules.
  • Note the source — some conditions end when the source is defeated or moves out of range.
